SENIORS WELLNESS PROGRAM

A comprehensive geriatric assessment is designed for individuals 65 years or older to help identify any medical, psychosocial, and functional limitations of an older person in order to develop a care plan to maximize his/her overall health with aging.

The Nurse Practitioner will assess and evaluate an older person’s functional ability, physical health, memory/cognition, mental health, nutrition, immunization, sleep, bone health, and other socioenvironmental circumstances.  Further assessment, evaluation, and management can be done if any potential problem has been identified. The Nurse Practitioner may refer you to other healthcare providers (e.g. social worker, dietitian, pharmacist, physiotherapist, and/or occupational therapist) for further assessment or support/treatment as needed. The Nurse Practitioner will also work with your Primary Care Provider and keep them informed to provide the best care.

Regular follow-ups are also provided by the Nurse Practitioner.

Please bring your medications, including the ones you take over-the-counter to your appointment.

Our seniors program also comprises of groups and education services. An intake assessment is conducted by our Occupational Therapist who reviews your needs and is able to offer guidance on appropriate groups and programs. Please see below for descriptions of our current program offerings.

GROUPS

ADULT EXERCISE GROUPS 1, 2 & 3
Starting with Level 1, the client will learn the basic skills for exercise with progression to Level 3 where we introduce balance and other techniques to complement activities of daily living. All equipment is provided. Initial assessment is conducted prior to beginning the exercise program.

FALLS PREVENTION
This 4 week workshop includes the following:

  • Review of falls from 4 different perspectives: dietary, medications, aging and physical health, and environmental/behavioural factors
  • Each session also includes light exercise led by our physiotherapist to encourage movement in a safe manner

HEALTHY LIVING & MEMORY GROUP
This group is intended for individuals seeking information regarding healthy aging and brain function. Topics reviewed include the following:

  • Introduction to the brain and is structure
  • Basic memory processes
  • Review of memory types
  • Lifestyle factors affecting memory
  • Review of memory tools to improve memory

Various games, activities and methods are also introduced throughout the group sessions to showcase the variety of ways one can practice engagement of a healthy brain.

SENIORS SOCIAL CLUB
This bimonthly group runs Fridays from 10am-12pm and is a great way for you to interact with other seniors within the community. The first hour of the group comprises of a variety of topics from health information to legal advice to a review of driving techniques with our local police department. The second hour comprises of light exercise with our physiotherapist. The group is held in the basement of the building and light refreshments and coffee are offered. Drop-ins welcome!
