Program Results

The “Home To Stay Program” (HTS) is a health initiative started on April 2017 that aims to manage patients’ health at home, enhance quality of life, and reduce unnecessary hospital visits. As of September 2017, all patients who were enrolled into the program had a reduction in avoidable ER/hospital visits.

The Pain Management Program is designed for people living with pain with the primary goals to: decrease pain and improve activities of daily living, prevent re-occurrence of pain for those with acute pain, and improve self-management skills for clients living with chronic pain. As of September 2017, 85% of patients were observed to have improved range of motion by the end of their 1:1 intervention with the Physiotherapist.

The Hypertension Program focuses on clinical and educational interventions aimed to lower blood pressure for patients whose blood pressure is above target and to optimally manage patients whose blood pressure is stable. Our results at the end of September 2017 indicated that 94% of patients showed an improvement in their blood pressure since they were enrolled.
